Member Handbook

 

The FHP Member Handbook provides easy-to-understand information regarding the Prepaid Mental Health Plan. The handbook is available in both English and Spanish. Our Member Handbook explains how to access services from Florida Health Partners, Inc. Information pertaining to our Provider Network and their service locations are listed.

The handbook also includes information about:

  • The terms and conditions of enrollment in the plan;
  • Information which indicates that members may select an alternative direct service mental health professional within the plan if one is available.
  • A description of services covered including limitations, exclusions and out-of-plan use;
  • A description of emergency mental health services and procedures available both in and out of the FHP service area;
  •  Procedures for obtaining mental health care services, including an explanation of the applicable restrictions of the plan.
  • Information which indicates that members must use the FHP network providers and secure appropriate referrals and authorization before receiving out-of-plan care;
  • Information regarding a member's right to request one of the covered services when the plan offers another service, not covered by the contract, as a downward substitution;
  • A description of grievance procedures and fair hearings, including telephone numbers of each service location and information about Medicaid’s Statewide Consumer Hotline;
  • A description of disenrollment rights and procedures, which includes information on the Medicaid Options contractor;
  • Member’s rights and responsibilities
  • Information on emergency transportation and non-emergency transportation available under the plan;
  • Information regarding Advance Directives and Living Wills
  • Information on the Title XXI KidCare program for those children between the ages of zero up to the age of 19, who become ineligible for Medicaid. Including information on how to apply for the Title XXI KidCare program in the event of a child losing their Medicaid eligibility and plan coverage benefits.
  • Information about telephone resources for the hearing impaired and information for resources to contact for assistance when necessary due to physical disabilities.
  • Information regarding the process for accessing translator services when necessary.

All members have the right to request and obtain information regarding the Prepaid Mental Health Plan.
