- How do I know if I am enrolled with Florida Health Partners, Inc. (FHP)?
- If you live in Pinellas, Pasco, Hardee, Highlands, Hillsborough, Manatee, Polk, Orange, Osceola, Seminole or Brevard counties and are enrolled in MediPass for your medical/physical health benefits, you will receive your mental health benefits through FHP.
- How do I get Care?

There are three ways you can get mental health care:
- Call or visit a network provider
- Call the FHP Information Line for Area 5: 1-888-563-9022, Area 6: 1-800-808-8033, or Area 7: 1-866-717-3816: 24 hours a day, 7 days a week.
- If you call your MediPass Primary Care Physician, he or she will help you get in touch with FHP to arrange for care.
- What do I do in an emergency?

If your need is an emergency, and you feel like hurting yourself or others, or when you feel unable to take care of yourself, you should:
- Go to the nearest network provider; or
- Go to the nearest hospital emergency room; or
- Call the FHP Information Line for Area 5: 1-888-563-9022, Area 6: 1-800-808-8033, or Area 7: 1-866-717-3816: 24 hours a day, 7 days a week.
- What information should I have ready when I call?
- You will be asked for some basic information, such as your name, address, zip code, county, phone number, Medicaid number, and any other health insurance information.
- Will I receive a list of providers?
- Your new member handbook contains a list of network provider locations with addresses, telephone numbers and office hours.
- Will my medical doctor know about my mental health care?
- Your MediPass Primary Care Physician will be notified of your treatment when you have signed a release of information. We encourage you to communicate and allow your family care doctor to participate in your care.
- Will getting mental health services cost me money?
- No. It will not cost you any money to receive covered Medicaid mental health services.
- Will my call and care be kept confidential?
- Yes, all of your records will be kept in strict confidence. We will not release any confidential information without your written permission except when required by law. We will need a signed release from you before we can discuss your treatment with anyone other than your mental health provider.
- What should I do if I am not happy with the mental health services I receive from FHP?
- If you are unhappy with your provider or some other service offered by FHP, please call the FHP Information line for Area 5: 1-888-563-9022, Area 6: 1-800-808-8033, or Area 7: 1-866-717-3816.
- Who are the providers in the Florida Health Partners, Inc. (FHP) network?

- Who do I contact to join the FHP Network?
- Send a written request to the Provider Services Department at the FHP Service Center, 3014 N. U. S. Highway 301, Suite 1000, Tampa, FL 33619. Please indicate your interest in becoming a network provider along with your specialty and age groups treated (child, adolescent, adult, geriatric).
- If I currently treat a Medicaid client in the FHP program, may I continue to be their provider?
- No. If you are currently seeing Medicaid members who will be enrolled with FHP for behavioral health care services, the member will be transitioned to a network provider in accordance with the transition schedule.
- What is the definition of emergency?
- Those services required to meet the needs of an individual who is experiencing an acute crisis, resulting from a mental illness, which is at a level of severity that would meet the requirements for involuntary hospitalization pursuant to section 394.467, Florida Statutes, and who, in the absence of a suitable alternative, would require hospitalization.
- Do I need a preauthorization to provide emergency services?

No. However, you must notify the FHP Service Center within the following guidelines:
In-area, contracted facilities:
These facilities are required to notify the Service Center within 4 hours of a person presenting for emergency mental health services. FHP is then required to respond within 4 hours of the notification as to whether or not it will authorize or deny admission. If FHP fails to respond within 4 hours, the facility can provide medically necessary services at its discretion and bill FHP for those services according to the FHP established fee schedule.
In-area, non-contracted facilities:
These facilities are required to notify the Service Center within 4 hours of a person presenting for emergency mental health services. FHP is then required to respond within 4 hours of the notification as to whether it will authorize admission or provide transportation to an in-network provider. If FHP fails to respond within 4 hours, the receiving facility can provide medically necessary services at its discretion and bill FHP for those services.
FHP is obligated to pay only for emergency services by non-network providers and those services will be paid according to Florida State Stautes. Any other services must be preauthorized.
Out-of-area facilities (outside of the Medicaid Area (5, 6, and 7)):
These facilities are required to notify the Service Center within 24 hours of a person presenting for emergency mental health services. FHP is obligated to pay for only emergency services provided by out-of-area facilities and those services will be paid according to Florida State Statutes. Any other services must be preauthorized.

- When will I receive payment for my services?
- FHP will process your claim within 35 days from the day of receipt of the claim form, with all required information (UB92 or CMS 1500)
